SUMMARY

     Hepatitis E is caused by infection with the hepatitis E virus, a non-enveloped, positive-sense, single-stranded RNA virus,32-34nm in diameter. Virus particles with a morphology reminiscent of HEV can be demonstrated by immunoelectron microscopy. Hepatitis E virus can infect humans, Various monkey species such as chimpanzees, owl monkeys, cynomolgus monkeys, rhesus monkeys, stump-tailed monkeys, African green monkeys, tamarins, marmosets and squirrel monkeys by the fecal-oral route, and contaminated water or food supplies have been implicated in major outbreaks. Symptomatic HEV infection is most common in young adults aged 15-40 years . Patients infected with HEV present with nausea, vomiting, anorexia,jaundice, abdominal pain, fever, and hepatomegaly. The clinical presentation of HEV is similar to that of acute hepatitis A.

The virus accumulates in high concentrations in the bile and is excreted via stool. Humoral immune response occurs providing diagnostic clues to the presence of HEV. IgM-anti HEV precedes the development of IgG by a few days and persists for four-to-five months after resolution of the acute illness. IgG anti-HEV persists for four-to-five years or longer and serves better as a marker for prior infection rather than acute infection(Figure1). Tests for IgM and IgG anti-HEV antibodies are commercially available.
